Please take a few moments to complete the following forms. By doing so, you will allow us to begin communicating you and your family concerning the services you would like. You will also enable us to contact your insurance company to obtain authorization for your services, contact your doctor's office (if needed) to obtain any additional reports, and allow us to better serve you. For your protection and security; information you enter will be encrypted and safely sent directly to our office once you complete all sections.

Navigating away from this form system or allowing it to “Time Out” before clicking "Submit" will cause the dismissal of any entered data. After successful submission you will be redirected to a Confirmation/Thank you page.

 

Please note: 

** As of April 23, 2025 we have a waitlist for home-based services in all areas. The list of open time slots for clinic-based services is available on the front page of our website (www.bdshelps.com). However, please be advised that your insurance company may not cover clinic based services. If you have any questions, please feel free to contact us.

By completing the following form, you will be added to our Future Family email listing as well as our newsletter to help keep you updated and provide you with additional resources. Please make sure that you submit all requested clinical documents as not submitting them will slow down pre-authorization and appointment scheduling. 

** We no longer send out a calendar links, or a recording prior to services. 

** We are in network with Aetna, CareFirst BCBS, Evernorth Behavioral Health/Cigna, JHHC/US Family Health, Maryland Medicaid, and United Behavioral Health/Optum.

** We are credentialed, but non-participating (non-network) with Tricare.

** We no longer provider services under Maryland's Home and Community Based Waiver for Children with Autism (the Autism Waiver).
